Quality of Care for Pregnant Services of Medical Genetics Cuba

Abstract

Introduction: The Cuban health system's strategic objective is to carry out efficient and sustainable actions to increase the quality of services. The perception of users about the performance of services is considered relevant in order to evaluate their quality.

Objective: To evaluate the quality of care perceived by pregnant women in prenatal medical genetics services in Cuba.

Methods: A descriptive, cross-sectional evaluation study was carried out in health systems and services. Sample composed of 1800 pregnant women from 11 provinces, interviewed in medical genetics services. A semi-structured questionnaire was used, validated by experts, with 22 items and an open question to collect suggestions. Variables grouped into domains: service location; socio-demographic characteristics and perception of quality, subdivided into dimensions: tangible elements, opportunity and interpersonal relationships. The summary variable “perceived quality” was obtained, resulting from scores of corresponding variables recoded into a qualitative scale.

Results: 60.7% of the respondents were in municipal medical genetics services, 22.4% in provincial services, and 16.9% in polyclinics. The respondents perceived good quality of care (96.7%), highlighting good interpersonal relationships (98.4%) and availability of diagnostic tools (96%). Difficulties were identified with structural problems (16.9%), distance from diagnostic services (22.5%), long waiting times (27.9%) and inadequate handling of privacy (9.2%).

Conclusions: Despite the good perception of quality by the interviewees, gaps are identified, which allow suggesting strategies for improving quality, in order to achieve excellent care. Keywords: Quality of Health Care; Prenatal Care, Cuba, Medical Genetics, Health Services, patient perspective.
