Evolution of the National Action Protocol for COVID-19 in Cuba: Patterns of Care During the Pandemic

Abstract

Introduction: In Cuba, the management of COVID-19 is summarized in the National Action Protocol with six versions and several key elements.

Objective: Characterize the evolution of the National Action Protocol with its care patterns.

Methods: Descriptive observational study with documentary review, nominal group in which was used the Del-phi method to define the main elements of the diagnosis that determined the patient´s evolution and the patterns of care.

Results: The National Action Protocol was framed in four stages. The first stage grouped the four initial versions. The second stage, was characterized by the improvement of the risk classification, contained version 5; The third stage, with version 6, included an enrichment of the epidemiological control, the improvement of the indications and the start of home isolation of contacts; And the fourth stage was distinguished by vaccination where version 6 was maintained with modifications for management in services, especially in Primary Health Care. There were defined four patterns of care, according to the degree of severity of the disease: mild (Pattern 1), moderate (Patterns 2 and 3, one without signs of systemic inflammatory disease and another with signs) and severe (Pattern 4).

Conclusions: The National Action Protocol with the medical care patterns used was consolidated at each stage and showed an evolution that could be related to the recovery rate of COVID-19 in Cuba, which must continue to be investigated.
