Health system reforms: an approach from management

Abstract

Introduction: The characteristics of health systems and services are linked to the level of development of each society, the political context, the economic situation, the advancement of medical sciences, the struggles of the most unprotected sectors, and community awareness of the problem. The need to introduce reforms in such a way as to influence the development of health systems is imposed, even if the results of these are not even.

Objective: To show fundamental concepts related to the reforms of health systems from a managerial approach.

Methods: A literature review was conducted for which it was carried out a search for scientific articles from journals indexed in the Medline (PubMed), SciELO, Scopus Medscape databases, articles, doctoral theses and printed books that collected the best evidence on the subject. Google Scholar search engine and the descriptors "health care reform" and "national health systems" were used.

Conclusions: Proposals for the reform of national health systems are often adorned with phrases that try to demonstrate their desirability, such as rationalization, simplification, financing, efficiency, among others. Health professionals, health leaders and citizen representatives must be assured about what the true meaning of these concepts is and to what extent they will produce a more just, equitable and sustainable system.
